Chronic Conditions Management

Occupational therapy (OT) plays a vital role in helping people live well with chronic conditions. A chronic condition is one that is expected or has lasted longer than 6 months.

  • Get back to the things you want to do in your home and community

  • Understand the aged care system and how to access help at home
